Well I do not know if you would call it a blooper but it creeped me out. 10 minutes 39 seconds into the movie "Event Horizon" where the Dr. has a hallucination of his dead wife on the ship. There is a set of eyes staring up at you from the console on the ship. it is not the Dr or the women. Its really creepy since none else is in the room but them. If you own the DVD look for it when you watch it again.:D
